• Supervise and assist residents with activities of daily living.
• Serve meals to residents and assist with feeding as needed.
• Provide person-centered care based on each resident's care plan.
• Take and record vital signs.
• Perform patient care as directed by the nurse safely according to policy and procedure.
• Respond to patient requests and communicate the patient's needs.
• Assist in transferring residents safely.
• Document care records for each resident each shift.
• Ensure cleanliness and dignity of every resident.
• Communicate effectively with residents, families, and staff.
