Job summary

Rochester Regional Health seeks a Certified Nursing Assistant (CNA) for the Dementia Unit at Clifton Springs Nursing Home, Clifton Springs, NY. This per diem, as-needed role supports the nursing team with resident care and companionship according to individualized care plans.

The CNA will assist with activities of daily living, promote safety, and document care while communicating with families. NYS certification and a minimum age of 18 are required; strong customer service is a plus.

Qualifications

• Must be a NYS-certified CNA.
• Minimum age 18 or older.
• High school diploma or equivalent.
• Excellent communication and customer service skills.

Responsibilities

• Patient Care & Service: Provide and/or assist residents in activities per individualized care plans; transport to activities; provide leisure activities; report and document; offer emotional support to residents and families.
• Safety: Maintain a clean and safe resident environment.
• Communication & Documentation: Support families, direct them to appropriate staff; respond to resident requests; relay reports to the team.
