Responsibilities

• Assist participants with daily living activities and hygiene.
• Support ambulation and mobility; assist with transfers as needed.
• Prepare meals and assist with prescribed diets for participants.
• Assist participants to bathroom or with bedpans/urinals as required.
• Consult daily with PCS Coordinator to update status and plan of care.
