Job summary

Suncoast Hospice, a member of Empath Health, seeks a caring CNA to provide personal care and ADL assistance for patients in their homes across North Pinellas County. Schedule is Monday–Friday, 8am–5pm, with a mission to offer Full Life Care and compassionate support to patients and families.

Qualified candidates hold a Florida CNA license, with 1–3 months of related experience preferred, and reliable transportation to service field-based visits.

Qualifications

• Experience: 1–3 months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA).
• Reliable transportation with proof of auto insurance for field-based tasks.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living.
• Assist patients with self-administered medications that are pre-poured as per the Plan of Care.
• Report changes in patient condition to the RN/Counselor and document communications.
• Participate in closure contact with family/caregivers after a patient's death as needed.

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and tasks related to the activities of daily living for patients in their homes in North Pinellas County on the following schedule Monday - Friday from 8am to 5pm.

What you'll Do

• The provision of hands‑on personal care and assistance with activities of daily living.
• Assisting patients with taking medications that are ordinarily self‑administered and have been pre‑poured as designated in the Plan of Care.
• Hospice Aides must report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care. Hospice Aide must document the changes and to whom they were reported to.
• Following a patient’s death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.

What You’ll Need

• Experience: One to three months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
