Ebenezer Ridges Care Center in Burnsville, MN is seeking a Certified Nursing Assistant (CNA/NAR) to support residents in a skilled nursing facility. The role involves hands-on care with bathing, grooming, and daily activities, following a resident care plan.

Responsibilities include documenting vitals, performing peri care and catheter care, assisting with transfers, and communicating resident needs to the care team.

Qualifications

• Active Nursing Assistant, Registered (NAR) on the MDH registry by start date.
• Ability to perform personal cares and monitor vitals as needed.

Responsibilities

• Assist residents with bathing, oral hygiene, grooming, toileting, dressing, showers.
• Check and record vitals per care plan.
• Provide AM/PM/NOC cares including peri care and catheter care.
• Turn, lift or transfer residents with/without assistance.
• Document observations in the electronic medical record system.
• Respond to call lights and communicate with staff.
