Dignity Home Health and Hospice is seeking a CNA Coordinator in American Fork, UT. The role coordinates and schedules CNAs to care for home health and hospice patients, while supporting field staff and ensuring accurate documentation.

Strong communication with caregivers, patients, families, and the interdisciplinary team is essential. The position requires an active Utah CNA certificate, CPR certification, and a valid Utah driver license with car insurance.

Qualifications

• Active Utah Certified Nursing Assistant certificate.
• Current CPR certificate.
• Valid Utah driver license with car insurance.
• Experience coordinating staff or scheduling (preferred).
• Excellent communication with caregivers, staff, patients and families.

Responsibilities

• Coordinate and schedule CNAs to care for home health or hospice patients.
• Support field CNA staff by covering visits within CNA training limits.
• Complete all documentation and charting accurately and timely.
• Oversee timely and accurate CNA charting submission.
• Communicate effectively with caregivers, employees, patients and families.
• Interact with patients, families, physicians, and interdisciplinary team members.
