Job summary

North Big Horn Hospital District seeks a Certified Nursing Assistant to support nursing staff in a SNF/ALF setting. You will assist residents with daily living activities, document care, and help with meals and mobility while following established care plans.

You will monitor weight, skin condition, and report changes to the nurse. The role requires English fluency, reliability, and ability to lift heavy items as needed.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit (when applicable)
• Must have no disciplinary action in effect against certification.
• At least one year of experience as CNA in SNF or ALF preferred, not required).

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to patients at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing patients according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident's skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done.
• Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned.
• Reports changes in a resident's condition to the resident's nurse and supervisor.
