Qualifications

• Current TB skin test or Chest X-ray required.
• Valid driver's license if assigned to a driving case.

Responsibilities

• Provide medical and personal care for clients based on their Plan of Care.
• Orient new caregivers to the clients’ care plans.
• Communicate changes in client medical status to Nurse Supervisor.
• Assist clients with transportation to appointments and activities.
• Document care activities accurately and timely.
