Job summary

Heritage-Woods-of-Mt.-Vernon is seeking a compassionate CNA to provide individualized care to residents as outlined in their care plans. You will assist with ADLs, promote independence and dignity, and report health changes to the clinical team.

Engage with residents and families in a caring, respectful manner, and participate in ongoing learning opportunities and staff meetings to ensure high-quality care and confidentiality at all times.

Qualifications

• High School diploma or GED required.
• Current CNA certification from an accredited program.
• Exceptional communication and interpersonal skills.
• Ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care according to each resident’s care plan.
• Assist with Activities of Daily Living while promoting independence and dignity.
• Report health status changes to the clinical team.
• Engage with residents and their family members compassionately to create positive experiences.
• Adhere to confidentiality and privacy regulations; participate in educational seminars and team meetings.
• Perform other duties as assigned.
