Job summary

• Must meet all applicable state and federal requirements for this position.
• Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility.

Responsibilities

• Provide physical support to assist residents with daily living activities such as lifting, transfers, bathing, dressing, toileting, standing, walking and meals.
• Observes and documents health and emotional condition, reporting changes to the nurse.
• Responds promptly to resident call signals and determines needs.
• Promotes safety and infection control; maintains clean and safe environment.
• Communicates with nurses regarding resident conditions, changes and concerns.
• Completes required documentation in a timely manner.
• Provides companionship, comfort and support to residents.
• Meets federal and state in-service requirements.
