Spring Creek Rehab is seeking a compassionate Certified Nursing Assistant (CNA) to join our team and provide high-quality patient care to residents. You will assist with daily activities, mobility, and coordination with nursing staff to ensure excellent care outcomes.

The role emphasizes collaborative care, ongoing assessment, and engagement with residents, families, and the care team in a supportive, patient-centered environment.

Qualifications

• Current CNA certification required.
• High school diploma or equivalent required.
• Previous long-term care experience preferred.

Responsibilities

• Provide direct patient care, including bathing, dressing, grooming, and feeding patients.
• Assist patients with mobility, transfers, and ambulation.
• Monitor and report changes in patient condition to the nursing staff.
• Develop and maintain positive relationships with patients, families, and staff.
• Participate in interdisciplinary care conferences and contribute to the development of patient care plans.
