Job summary

Landmark Assisted Living in Vernal, UT is seeking a Certified Nursing Assistant to provide direct resident care in accordance with care plans and facility policies. This position is non-exempt and hourly, reporting to the Resident Care Coordinator or designee.

The role requires a valid CNA license, current CPR/BLS, and ability to assist with activities of daily living, mobility, nutrition support, and documentation. You will join a compassionate team dedicated to safe, dignified resident care.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit (when applicable)
• Must have no disciplinary action in effect against certification.
• Skilled Nursing or Assisted Living Facility: At least one year of experience as a certified nursing assistant in a SNF or ALF is preferred, not required)

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to patients at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing patients according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.
