Responsibilities

• Assist with mobility and transfers (walking, standing, lifting).
• Support with incontinence care, medication administration, and essential tasks.
• Collaborate with the Interdisciplinary team to implement individualized care plans.
• Maintain accurate records of patient care and progress.
