Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and personal care.
• Assist residents with mobility, transfers, positioning, and ambulation as needed.
• Take and record vital signs and report changes or concerns to the nursing team.
• Respond promptly to resident call lights and requests for assistance.
• Assist residents during mealtimes and with feeding as needed.
• Observe residents and communicate changes in condition, behavior, or needs to the appropriate nursing team member.
• Provide companionship, reassurance, and emotional support to residents.
• Maintain accurate and timely documentation of resident care.
• Help maintain clean, safe, and comfortable resident rooms and common areas.
• Follow established infection control, safety, and resident care procedures.
• Work collaboratively with nurses, caregivers, and other team members to provide exceptional resident care.

About the Role

Plum Creek Chicago is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our team.

As a CNA, you will provide hands‑on care and daily support to residents while promoting dignity, independence, comfort, and quality of life. You will work closely with our nursing team to ensure each resident receives attentive, individualized care in a safe and supportive environment.
