Job summary

Phcwoods in Terre Haute, IN is seeking CNAs for full-time/part-time night shifts. Responsibilities include assisting residents with ADLs, feeding, vital signs, mobility, and safety. The role requires documentation, teamwork, and strong communication to ensure quality care.

Night shift differential is available; basic medical equipment handling and infection control are part of daily duties. Experience in long-term care is preferred by the facility.

Qualifications

  • Ability to read/write comprehensive English and document observations.
  • Must perform basic math.
  • Ability to verbally communicate with residents, staff and public clearly.
  • Must safely perform essential functions with or without accommodation.

Responsibilities

  • Assist with Activities of Daily Living including bathing, dressing, grooming.
  • Serve and collect meals; assist in feeding residents.
  • Take and record vital signs and weights.
  • Turn and reposition residents; assist mobility with devices.
  • Transport residents using wheelchairs, walkers, lifts.
  • Maintain safety and infection control; report hazards promptly.
  • Document care in resident records and facility forms.
  • Communicate changes to charge nurse; collaborate with team.
  • Maintain clean, orderly resident units; use approved items.

Description

SCHEDULE: Full-Time or Part-Time, Night Shift

HOURS: 6:00 PM – 6:30 AM

NIGHT SHIFT DIFFERENTIAL: Available

AREAS OF RESPONSIBILITY
A. Provide Assistance with the Activities of Daily Living
  1. Provide daily hygiene measures including bathing, oral hygiene, skin care, hair and nail care, as assigned and as required.
  2. Change bed linens as required.
  3. Dress and undress residents as needed.
  4. Serves and collects food trays.
  5. Assist in feeding and providing oral fluids to residents.
B. Assist Residents in Preventive, Therapeutic, and Restorative Nursing Procedures
  1. Take and record vital signs and weights.
  2. Turns and repositions residents who require assistance.
  3. Transports residents using wheelchair, walkers, lifts and other assistive devices.
  4. Perform range of joint motion exercises daily as needed.
  5. Assist residents to ambulate, or use assistive devices.
  6. Assist the resident in bowel and bladder elimination activities and retraining programs.
C. Adhere to Safety and Infection Control Policies and Procedures
  1. Maintain good personal hygiene and adhere to uniform requirement.
  2. Practice standard precautions.
  3. Perform cleaning of resident bedside unit, care equipment, and utility room.
  4. Provides for the safety of the resident using safety devices identified on the assignment sheet.
  5. Report personal illness or skin lesions.
  6. Participates in orientation training and twelve (12) hours of in-service programs annually.
  7. Report defective equipment or other potential safety hazards promptly.
  8. Adheres to Nursing Department policies and procedures.
D. Communicate Effectively Orally and in Writing
  1. Report physical or behavioral changes in the resident, accidents/incidents immediately.
  2. Document resident nutrition and fluid intake and outputs.
  3. Document daily on facility forms and in resident's medical record.
  4. Clarify questions regarding assignment with charge nurse.
  5. Report all grievances and complaints appropriately.
  6. Develop solutions using problem solving skills.
  7. Control angry feelings.
  8. Maintain and safeguard entrusted confidential information.
  9. Be flexible, receptive and adaptive to change.
  10. Exhibit teamwork with all employees and hold others accountable in delivering the best care.
E. Maintain Resident Unit and Nursing Work Areas in Clean, Orderly, and Safe Manner
  1. Use only approved items.
  2. Replace furniture to proper safe location.
  3. Handle equipment and supplies to avoid wastage or breakage.
  4. Use time effectively and efficiently, asking for help or notifying supervisor of uncompleted tasks.
Qualifications
  1. Ability to read/write comprehensive English and document observations.

  3. Ability to verbally communicate with residents, staff and general public in a manner that is understandable, must be honest, dependable and exhibit a warm, cheerful, caring manner.
  4. Must be able to safely perform the essential functions of the job with or without reasonable accommodation.
Education and/or Experience
  1. Current CNA Certificate in good standing on file with the Indiana Nursing Assistant Registry.
  2. One year experience in long-term care or other health care setting desired.
Physical Demands
Physical
  1. Will need to continuously stand and walk, squat, bend to floor, reach, twist, sit and potentially remain in uncomfortable positions for extended periods of time.
  2. Must be able to lift, move and turn residents. Requires varying degrees of pushing, pulling and lifting depending on the weight of the resident, which is often times over 100 pounds.
  3. Must have eye/hand coordination sufficient to assist residents with ADL's, transfers, and ambulation.
  4. Must be able to clearly speak.
  5. Must deal calmly with multiple tasks, noises, and interruptions on a continuous basis.
Sensory
  1. Must be able to hear normal sounds and voice patterns with constant background noise.
  2. Must be able to receive and follow verbal instructions.
  3. Must be able to respond to audible emergency signals, alarms, call lights and telephones.
  4. Must be able to smell smoke, spoiled food, incontinent residents, etc.
  5. Must be able to identify a resident within a reasonable distance (20 feet), focus and read thermometer, oxygen pressure gauge and similar equipment.
  6. Ability to identify colors for warning lights, signs and labels.
  7. Must be able to read regular/small size print.
Cognitive
  1. Must be able to remember multiple assignments over an extended period.
  2. Must be able to remember specific requests, ideas and concepts and relate them as necessary.
  3. Must be able to concentrate on details during constant interruptions.
