Responsibilities

• Perform personal care needs and attend to patient requests promptly
• Report to the Case Manager if unable to perform tasks
• Communicate positively with patients and families, maintaining confidentiality
• Ensure safety using equipment properly
• Provide personal care such as baths, oral hygiene, shampoos, and linen changes
• Assist with dressing, undressing, and meal preparation
• Assist with feeding as assigned
• Take and record vital signs and monitor patient condition
• Prevent skin breakdown and report significant changes
• Assist with ambulation, exercises, and transfers
• Support medication reminders and light laundry tasks
• Participate in quality improvement activities
• Maintain a respectful environment and adhere to documentation standards

Qualifications

• Meets state training requirements
• Completed at least 75 hours of classroom and supervised practical training
• Minimum six months experience providing personal care
• At least 18 years old
• Ability to read, follow instructions, and document care
• Understanding of hospice philosophy and comfort with terminally ill care
• Good references, CPR certification, and ability to work independently
• Calm demeanor and good communication skills
