We are seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our team at Tucker House Nursing and Rehabilitation, located in Philadelphia, Pennsylvania. As a CNA at Tucker House, you will play a vital role in providing exceptional care to our residents, promoting a warm and homelike environment that feels like a true home.

About the Role:

• Assisting with personal care activities, such as bathing, dressing, and grooming
• Supporting resident mobility and transferring residents safely
• Assisting with mealtime and hydration
• Monitoring and reporting changes in resident condition
• Collaborating with the interdisciplinary care team to develop and implement individualized care plans
• Providing emotional support and companionship to residents and their families

Responsibilities:

• Provide direct patient care to residents, including vital signs, wound care, and medication assistance
• Document resident care and progress accurately and timely
• Participate in resident care conferences and contribute to the development of individualized care plans

Requirements:
