+ **Department:** Skilled Nursing

+ **Schedule:** Full-Time Evenings 2p-10:15p & Part-Time shifts (6a-2:15p, 2p-10:15p) 8 hour shifts & will include rotating weekends/holidays

+ **Facility:** Saint Anne Place

+ **Location:** 4405 Highcrest Road Rockford, IL 61107

+ **Salary Range:** $17.31 - $22.30 per hour

**Responsibilities**

Assist with basic patient care activities.

+ Assist with procedures ordered by physician and supervised by a Registered Nurse (RN).

+ Assist patients with tending to personal care, activities of daily living and transfers/transport.

+ Report findings or changes in physical, mental and emotional conditions to nursing staff.

+ Assist with keeping patient rooms clean and orderly. Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.

+ Assist in providing care appropriate to a specific population by facilitating time and individualizedcare of assigned patients.

**Requirements**

Licensure / Certification / Registration:

+ Nurse Aide obtained prior to hire date or job transfer date required.

+ BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

+ High school diploma or GED preferred.
