Qualifications

• High school diploma or GED is required.
• Nursing Assistant I certification by the North Carolina Department of Health and Human Services.
• Crisis Prevention Institute (CPI) certification within 60 days from date of hire.
• Basic Life Support (BLS) Certification.
• Requires one year of relevant experience in a behavioral health environment.

Responsibilities

• Aids patients in reality orientation and participates in behavior modification plans; provides informal support to patients and families; verbal de-escalation of potentially aggressive patients.
• Interviews patients and families for admission data and orients them to the facility; contributes data input during team discussions to develop therapeutic plans.
• Interacts with patients one-to-one and in groups; facilitates assigned groups to maximize participation appropriate to group members.
• Observes and documents patient information; reports significant changes to a registered nurse or physician.
• Provides direct personal care including bathing, hygiene, feeding, dressing, positioning, ROM, transfers and walking; records vital signs, intake/output, and progress notes.
• Ensures patient safety, participates in protective restraint when necessary, and monitors patients in seclusion or restraint as needed.
• Reinforces routine teaching and distributes patient education materials.
