Focused Post Acute Care Partners (FPACP) is seeking a Certified Nurse Aide to join our team at Focused Care at Clarksville in Clarksville, TX. The position is full time.

Position Summary

Responsible for assisting residents with activities of daily living to promote resident independence and dignity.

Qualifications

• High school diploma or equivalent (GED)
• Current Nurse Aide certification in the state of Texas
• Good standing on the Nurse Aide Registry and Misconduct Registry in Texas
• Ability to understand, remember, and carry out verbal and written instructions in English
• Ability to speak clearly and hear well enough to communicate with residents

Essential Functions

• Ensure resident safety.
• Provide bathing, showering, shampooing, shaving, hair combing, dressing, nail care, and other personal hygiene for assigned residents.
• Follow resident assignment schedule as determined by the charge nurse.
• Lift, move, and transfer residents as required.
• Answer call lights promptly.
• Assist with feeding residents.
• Maintain resident cleanliness, dryness, and toileting or incontinent care as needed.
• Provide a supportive, protective, and safe environment for residents.
• Perform other duties as assigned.
• Utilize electronic medical records charting (e.g., eChart point‑click care).

Physical Requirements

Must be able to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds, with or without reasonable accommodations.
