Qualifications

• High School diploma or GED required.
• 105 hours CNA program required.
• Name must be on the Indiana CNA Registry at time of hire and maintained thereafter.

Responsibilities

• Assist with admission, transfer and discharge of residents as instructed by Charge Nurse.
• Assist residents with activities of daily living and personal care.
• Provide direct patient care as assigned by Charge Nurse.
• Document resident care accurately and promptly.
• Communicate changes in resident condition with Charge Nurse and leadership.
