Job summary

Lincoln Community Hospital and Nursing Home in Hugo, CO 80821 is seeking a Certified Nursing Assistant to provide routine daily nursing care and assist with rehabilitative services in a small, critical access facility.

The role supports both the 15-bed acute care and 35-bed long-term unit, with emphasis on compassionate patient care, accurate documentation, and participation in quality improvement activities. Must have high school diploma and CNA/BLS qualifications.

Qualifications

• Graduation from high school.
• Previous nursing assistant experience or student nurse preferred.
• Completion of certification program for nursing assistants preferred.
• Nursing Assistants can work for 4 months following the completion of a class before testing.
• Must obtain BLS within 6 months of hire and maintain per certification requirements.

Responsibilities

• Provides care as directed by the professional nurse to patients/residents requiring long-term, rehabilitative care or restorative care.
• Reports observations and other pertinent information related to the care of the patient/resident.
• Documents objective information related to patient/resident care.
• Assists with registration of patients/residents in the computer.
• Provides services that support the care delivered to the patient/resident.
• Provides input in the formulation and evaluation of standards of care.
• Participates in the development of unit objectives.
• Participates on the identification of staff educational needs.
• Conducts self in a professional manner in compliance with unity and facility policy.
• Participates in the quality assessment and improvement process and activities.
• Performs other related duties as assigned or requested.
