Southern HealthCare Management is seeking a compassionate Activity Aide to support an ongoing program that promotes residents' physical, mental, and psychosocial well-being. You will assist the Activity Manager, coordinate with volunteers, and help implement activities tailored to each patient's needs.

• Assists with volunteers and volunteer programs as requested.
• Assists with planned activity programs to meet patient needs.
• Records attendance and participation in activities accurately.
• Completes assessments, MDS, care plans and designs activities.
• Escorts patients to and from activities.
• Helps develop the Activity calendars.
• Provides personal care and ADLs as needed and related tasks.
• Obtains and records vital signs and weight when assigned.
• Documents all care per policy and regulations.
• Comforts patients, transports, responds to call lights, reports observations.
• Maintains confidentiality of patient information.
• Adheres to standards and policies to protect facility.
