CNA 1st shift 7a-3p with alternating weekends - Full time

POSITION SUMMARY:

The CNA/caregiver provides medical and physical care to residents, meeting their daily needs and enabling them to function at their highest level. Under the supervision of a professional nurse, the caregiver is the residents' closest advocate, promoting psychosocial well-being through relationship building, and collaborating with the household team to meet social service needs. The caregiver plans and facilitates social activities, supports household maintenance, and ensures safety and hygiene standards are met.

Essential Duties and Responsibilities:

  • Attend to residents' personal hygiene, grooming, nutrition, mobility, elimination, psychosocial, and safety needs, supporting those unable to care for themselves.
  • Support residents' restorative therapy plans under professional guidance.
  • Observe residents for changes in needs or conditions and report immediately.
  • Participate in the resident’s care plan, advocating on their behalf and respecting their preferences.
  • Collaborate with household team members to meet residents' needs and desires.
  • Seek opportunities to address residents' psychosocial needs through planned and spontaneous activities.
  • Plan and organize social activities, ensuring their success.
  • Facilitate spontaneous activities of residents' choice and accompany them during these activities.
  • Maintain equipment, games, and supplies supporting activities.
  • Document social service and activity-related information accurately.
  • Keep household spaces, including resident rooms and common areas, clean and organized.
  • Manage supplies to meet resident needs.
  • Support household environment aspects, including pets, plants, visitors, and activities.
  • Coordinate with the homemaker to meet residents' laundry needs.
  • Perform other duties as required, consistent with the position.

Education and/or Experience:

  • Graduate of a state-approved nursing assistant program, with competency evaluated, tested, and registered.
  • High school diploma or GED preferred but not required.
  • 2+ years of experience in a Skilled Nursing Facility, with dementia experience preferred.
