Job summary

Cura Tulsa Hospice is seeking a compassionate CNA to assist patients with personal care, mobility, and daily living tasks in home hospice settings.

You will follow written care plans, document services, and communicate changes to the RN or Clinical Director while maintaining a safe, respectful environment.

Qualifications

• High school diploma or GED required.
• CNA/CHHA/LTC license/certification required.
• Driver's license with valid auto liability insurance required.

Responsibilities

• Follows written instructions for patient care as prepared by an RN or Clinical Director in the hospice plan of care.
• Assists patients with personal care and mobility to preserve hygiene while maintaining a safe environment.
• Performs simple procedures as an extension of therapy services (ambulation, ROM, basic exercises).
• Reports changes in patient condition to supervisor or RN.
• Maintains record of services performed and current patient condition.
• Establishes and maintains a positive working environment with patient/family and hospice staff.
• Participates in patient care conferences to ensure continuum of care.

• Performs simple procedures as an extension of therapy services, such as ambulation, ROM and exercise.
• Reports changes in patient’s condition and needs to supervisor or RN.
• Maintains record of services performed and of apparent condition of patient.
• Establishes and maintains a good working environment with the patient/family and co-workers of the Hospice.
• Participates in patient care conferences to provide input requiring continuum of patient care.
• Perform household services essential to health care at home.
