Job summary

Traditional Home Health Care is seeking a dedicated CNA to provide personal care and ADL assistance to patients in Forest City, PA and surrounding areas. You will travel to multiple patient sites daily, delivering compassionate care in patients' homes, under supervision, and within a defined geographic territory.

Job Title: CNA

Shift(s): Monday - Friday 8:30am - 5:00pm (5-7 visits daily)

Job Location: Forest City, PA and surrounding areas

Our CNAs see patients in the most ideal setting - where the patient resides!

Our offices are located in Dunmore, PA . At Traditional, we do our best to schedule CNA visits within a sensible geographic territory every day.

Job Responsibilities Include, But Not Limited To The Following

• Administers personal care and ADL assistance to patients according to the individual's care plan.
• Reports patient status changes to nursing supervisor on a continuous basis.
• Prepares, maintains and submits documentation as required in a thorough and timely manner.

Requirements

• Current Pennsylvania certification as a Certified Nurse Aide
