Job summary

Four Oaks Healthcare Center in Jonesborough, Tennessee is hiring a CNA for day shift. Join a compassionate care team offering a $2,500 sign-on bonus and a supportive work environment focused on resident well-being.

The CNA will assist residents with daily activities, follow care plans, document meal intake, and report changes to nursing leadership while maintaining PHI confidentiality. Active Tennessee CNA and CPR/BLS certifications are required.

Qualifications

• Active Tennessee CNA certification
• Current CPR/BLS certification
• Ability to assist with activities of daily living
• Ability to document meal intake and care tasks

Responsibilities

• Provide supportive services and perform duties as assigned by clinical leadership
• Assist residents with activities of daily living in accordance to care plans
• Assist with patient care tasks and coordinate dining services including meal tray delivery, feeding assistance, and documentation of meal intake
• Report changes in a resident’s condition to the nurse supervisor and inform of factors affecting performance
• Maintain confidentiality of protected health information (PHI) in verbal, written, and electronic form

CNA Position Summary

• Provide supportive services and performs duties as assigned by clinical leadership.
• Assist residents with activities of daily living in accordance to care plans.
• Assist with patient care tasks and coordinate dining services including meal tray delivery, feeding assistance, and documentation of meal intake.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

CNA Requirements

• An active CNA Certification for the state of Tennessee
• Current CPR/BLS Certification
