Job summary
InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike focused on accessible, compassionate care in Connecticut.
The role centers on providing clinical services within the School Based Health Center, including individual and group therapy and trauma-informed care. The position requires a Master’s in Behavioral Health and CT licensure, with a schedule of Tuesday through Friday, 8:00 am–1:00 pm.
Qualifications
- Master’s degree in Behavioral Health or equivalent required with state licensure to practice clinically.
- Licensed in Connecticut for clinical social work or corresponding discipline is required or must be maintained.
Responsibilities
- Provide individual and group therapy within the school system for children enrolled in the SBHC.
- Provide education and training to school staff on trauma-informed care.
- Coordinate with school social workers, nursing staff, and administrators to support children.
- Develop psychosocial assessments through clinical interviews considering developmental, social, cultural, and medical factors.
- Develop treatment plans with measurable objectives.
- Deliver clinical treatment services including triage, crisis intervention, and family therapy as needed.
- Advocate and develop networks of social and clinical services to assist clients.
- Maintain treatment records and complete required documentation and reports.

Accountable for providing Clinical services within the School Based Health Center.
- Provide individual and group therapy within the school system to children who are registered with the SBHC
- Provide education and training to school staff around trauma informed care
- Coordinate with school social worker, nursing staff and other school administrators to ensure proper support is being offered to the children
- As a member of multi-disciplinary clinical team or within a network of community based behavioral health services develop psychosocial assessments through clinical interviews with assigned clients and families which include consideration of developmental, social and/or cultural, substance abuse, psychological, environmental and medical issues
- Develop treatment plans including measurable treatment objectives
- Provide clinical treatment services in form of triage, crisis intervention, individual, group and/or family therapy, screening and planning
- Advocate and develop networks of social and clinical services to assist clients in meeting identified needs
- Monitor and evaluate effectiveness of treatment plans, documents treatment in clinical records
- Complete all required documentation related to client treatment in accordance with established procedures
- Complete reports in accordance with agency and accrediting bodies
- Consult and collaborate with other behavioral health providers to improve delivery of services to clients
- Maintain treatment records in accordance with agency and regulatory bodies
- Perform related duties as required
- All agency staff are required to attend all mandatory department/agency meetings and trainings*
Tuesday - Friday, 8:00 am - 1:00 pm
Master’s Degree in Behavioral Health discipline from an Accredited University and Licensure in the State of Connecticut as a Clinical Social Worker, Marriage and Family Therapist, Psychologist, Substance Abuse Counselor. Incumbents in this class must retain a license to practice clinical social work or other discipline listed above.
