Job Description

The Clinician is responsible for the provision of direct services to each adolescent on their caseload, including the development of a comprehensive assessment for each client. In addition, the Clinician acts as a liaison with the client’s family, Department of Children & Families (DCF) Social Worker and outside agencies.

Program / Department Responsibility
  • Review referrals to determine appropriateness for placement within the program and respond to DCF within contracted timeframes.
  • Ensure a thorough intake of youth into the program has occurred, including a suicide screening/safety assessment, completion of MAYSI screening, and creation of a safety plan.
  • Complete CRAAFT, LITE, and CANS assessment within 7 days of placement with each youth and their identified family.
  • Develop an individual treatment plan for each youth and their identified family that includes the six domains and identifies all necessary therapeutic, medical, and dental care.
  • Provide intensive individual, group, and family therapy and review each youth’s progress toward identified goals at treatment reviews that meet licensing timelines.
  • Ensure referrals/appointments are made for youth in need of psychological evaluation, psychiatric services, therapy, dental, and/or medical appointments; oversee additional assessments as requested.
  • Assess and monitor all youths on suicide watch on an ongoing basis.
  • Maintain communication with parent/guardian/LAR and collateral agencies.
  • Attend and participate in program meetings (weekly clinical reviews, weekly house meetings, monthly staff meetings, and administrators meetings).
  • Track data and statistics in line with permanency and standards, as well as service referrals.
  • Provide rotating on-call clinical coverage and be available for after‑hours emergencies or intake.
  • Ensure proper accounting of youth belongings and valuables.
  • Follow behavior management practices on all shifts.
  • Work with caseworker to ensure all medical and dental needs are met in a timely fashion.
  • Support program staff with skill development and coaching needs.
  • Contribute to a positive work environment by displaying pride in job performance, enthusiasm, and cooperation with co‑workers.
  • Interface with, and provide support for, all clinical interns, community partnerships, staff, and volunteers regarding the programming of the DCF congregate care sites.
Facility / Housekeeping Responsibility
  • Maintain upkeep, cleanliness, and security of the Old Colony Y Program assigned site.
  • Maintain daily upkeep, cleanliness, and organization of office use and program equipment.
Interdivisional / Community / Volunteer Responsibility
  • Actively participate and provide leadership for the Y’s Annual Campaign by completing all required tasks.
  • Represent the Old Colony Y in a community‑based service club or other community organization.
  • Offer support in crisis situations to all Old Colony Y Programs.

Additional Considerations

There will be occasions that the use of your personal vehicle will be expected and required. You will be reimbursed for the mileage completed during an Old Colony Y assignment.

Closing Statement

To ensure the overall operation of the Y by supporting teamwork, the Clinician will accept additional responsibilities as assigned.

Education / Experience Requirements (Minimum Required)
  • Master’s Degree in Social Work, Psychology, or Human Services field required; LCSW preferred.
  • 1 – 5 years of related experience required.
  • Strong knowledge of Microsoft programs required.
  • Excellent reading and writing skills for contract and incident reporting.
Essential Job Functions
  • Communicate with clients, families, and collateral resources in person and by phone; daily attendance and scheduling flexibility.
  • Assist with restraining residents up to approximately 200 lbs.
  • Maintain visual and auditory acuity to support program security.
  • Stand, walk, and crouch intermittently.
  • Push, pull, and lift up to 25 lbs. on occasion.
  • Drive, maintain a safe driving record, and hold a valid driver’s license.
  • Pass a drug screen.
