BAYADA Home Health Care is currently seeking an experienced clinician (RN, LPN, OT, OTA, PT, PTA, or Social Worker) to fill the position of Transitional Care Manager (TCM). Clinical Liaisons at BAYADA interact with discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of home health care services.
This will be a hybrid role. The TCM will work remotely Monday/Friday, and will be on-site at Thomas Jefferson University Hospital Tuesday-Thursday, 8:30am-5pm.
