Job summary

GuideStar Eldercare in Minnesota seeks a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) to provide neurobehavioral health services to residents in long-term care facilities, collaborating with a multidisciplinary team.

You will support elderly individuals with neurocognitive disorders and other neurological or psychiatric conditions, ensuring continuity of care and holistic treatment.

Qualifications

• Master’s degree in Mental Health Counseling or Social Work is required.
• Licensed to practice LMHC or LISW in the applying state.
• Ability to collaborate within an interdisciplinary treatment team.
• Demonstrated time management and autonomous functioning with regional support.

Job Description:

As a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.

Required Qualifications:

• Ability to work as part of an interdisciplinary treatment team
• Good time management skills
• Able to function autonomously with regional Area Manager support
• Master’s degree in Mental Health Counseling or in Social Work
• Currently licensed as an LMHC or LISW in the state in which you are applying

Preferred Skills:

• Passion for geriatrics
• Experience in inpatient settings such as extended care facilities
• Experience with brief bedside assessment screens (cognitive, mood, etc.)
• Motivated by productivity-based positions
• Ability to navigate and document on an Electronic Medical Record (EMR) system
