Job summary

Lever, Inc. is seeking a Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) to provide neurobehavioral health services to residents in long-term care facilities.

You will work within a multi-disciplinary team to ensure holistic, continuity of care for elderly individuals with neurocognitive disorders. Responsibilities include evaluation, condition-specific therapy, and coordinating detailed care plans with facilities, PCPs, and GuideStar colleagues, including family

Qualifications

• Must be LMSW or LPC licensed in the applying state.
• Ability to work as part of an interdisciplinary treatment team.
• Strong time management and autonomous functioning with regional support.

Job Description

As a Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.

What you will do

• Provide evaluation


• Provide condition-specific support therapy, or when indicated, insight-based therapy


• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.

Required Qualifications

• Ability to work as part of an interdisciplinary treatment team


• Good time management skills


• Able to function autonomously with regional Area Manager support


• Master’s degree in Mental Health Counseling or in Social Work


• Currently licensed as an LMSW or LPC in the state in which you are applying

Preferred Skills

• Passion for geriatrics


• Experience in inpatient settings such as extended care facilities


• Experience with brief bedside assessment screens (cognitive, mood, etc.)


• Motivated by productivity-based positions


• Ability to navigate and document on an Electronic Medical Record (EMR) system
