Mercyhealth seeks a licensed behavioral health professional to perform psychosocial assessments, develop and implement treatment plans, and deliver therapy in collaboration with a multidisciplinary team. The role includes crisis intervention, discharge planning, and coordination with physicians and community agencies to ensure comprehensive patient care.

Responsibilities include charting, attending team meetings, and participating in ongoing performance improvement activities.

Qualificações

• Master's degree in counseling or related field and relevant experience

Responsabilidades

• Completes psychosocial assessments and treatment plans with patient and team
• Conducts comprehensive clinical needs assessment prior to therapy
• Collaborates with patient to develop treatment plans
• Delivers behavioral health care within license scope
• Works in treatment relationship with patients individually or in groups
• Provides crisis assessment and intervention
• Coordinates care with physicians and community agencies
• Discharges planning and referrals to resources
• Participates in supervision and performance improvement activities
• Maintains medical record documentation per guidelines
• Attends required meetings and staffing
