Qualifications

• Masters in a behavioral health field and Illinois licensure (LCSW or LCPC) are required.
• Experience in clinical behavioral health settings is preferred.
• Strong documentation and EHR proficiency required.

Responsabilités

• Conduct psychosocial assessments and behavioral health evaluations.
• Provide individual, family, and crisis intervention therapy.
• Develop and monitor treatment plans with measurable goals.
• Coordinate care with physicians, teams, patients, families, and providers.
• Maintain compliant EHR documentation and discharge planning.
