Job summary

Eventus WholeHealth is seeking a Clinician Support Specialist to assist APCs and facility staff with workflow coordination, documentation, and telehealth support. The role emphasizes patient care, communication across teams, and high-quality service in a fast-paced clinical setting.

You will travel to assigned facilities regularly, support encounter documentation and chart prep, and help with discharge planning while maintaining patient privacy and compliance.

Qualifications

• High school diploma or equivalent required.
• CMA/CNA/PCT certification preferred.
• Minimum 1 year of patient care experience preferred.
• Valid driver's license and reliable transportation.
• Excellent verbal, written, and electronic communication.

Responsibilities

• Prepare for daily clinical rounds with patient information, documentation, and schedules.
• Assist clinicians with encounter documentation, data entry, and chart prep.
• Coordinate and maintain accurate patient schedules including admissions/discharges.
• Serve as liaison between clinicians, facility staff, patients and families.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation and lab results.
• Host and support telehealth visits for routine and acute care.
• Obtain and process patient consents and route documentation properly.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and maintain accurate census and records.
• Represent the organization professionally within partner facilities.
