Job summary

Yale NewHaven Health's Bridgeport Hospital is seeking a Clinical Support Specialist who guides patients through the healthcare system and supports access to services. You will collaborate with physicians, staff, and the Navigation team to document information in the electronic medical record and coordinate referrals.

The role emphasizes culturally competent communication, independent work with supervision, and a patient-centered approach to improve care delivery in a busy clinical setting.

Qualifications

  • High School Diploma or GED required; Associates Degree in health/human services or related field preferred or medical assistant certification preferred.
  • At least 2 years experience in health care or human services with a focus on community setting, or 1 year with Associate's Degree.
  • Ability to work independently with supervision and collaborate effectively with patients, families, staff and providers.

Responsibilities

  • Schedule, confirm, coordinate, and provide support to patients for in-office and external appointments.
  • Contact patients by phone to remind of upcoming appointments, follow-up care questions, and provide instructions; reschedule as needed.
  • Communicate effectively with patients, families, facility staff, and colleagues.
  • Demonstrate cultural sensitivity and adapt to diverse patient populations.
  • Assist patients with access and adherence to care within the health system.
  • Enter and access data in the electronic medical record; complete referral-related administrative tasks.
  • Collaborate with Bridgeport Hospital Primary Care Center staff and Navigation team.

Clinical Support Specialist: Reports to the Manager of Care Coordination and works in collaboration with physicians, staff and other healthcare professionals. Help guide patients through the healthcare system and ensures that patients and members are able to access. Provides Culturally and Linguistically appropriate services in a timely and cost effective manner. Works closely with the patient care team and key collaboratives across the health system.

Responsibilities
  • 1.Schedule, confirm, coordinate, and provide support to patients for in office and external appointments.
  • 2.Contacts patients by phone to remind of upcoming appointment(s), to ask follow up care questions, and to provide special instructions if needed. Cancels and reschedules appointments as needed.
  • 3.Must be able to communicate with patients, families, facility staff, and colleagues effectively and professionally
  • 4.Display sensitivity to and experience working with different cultures.
  • 5.Display comfort and interest working with members around issues of healthcare access and adherence.
  • 6.Possess ability to multi-task and work under time pressures.
  • 7.Demonstrate computer proficiency inclusive of ability to enter and access data/information.
  • 8.Demonstrate ability to communicate both verbally and in writing with the members of the health care team.
  • 9.Demonstrate ability to be a self-starter and work independently.
  • 10.Completes all administrative functions associated with referral activities in a timely manner.
  • 11.Works closely with Navigation team to assist patients with external resources as needed.
  • 12.Ensures that all necessary participant information is documented in electronic medical records
  • 13.Works collaboratively with the Bridgeport Hospital Primary Care Center Staff.
  • 14.Performs other duties as needed.
Qualifications
EDUCATION

High School Diploma or GED required. Associates Degree in health and/or human services or related field preferred or medical assistant certification preferred.

EXPERIENCE

At least 2 years experience in health care or in human services with a focus on community setting in health care or in human services or 1 year experience or related internship in health care or human service with a focus on community setting with an Associate's Degree.

SPECIAL SKILLS

Ability to work independently with support and supervision from Manager of Care Coordination and Director of Community Health and to also collaborate and communicate effectively with others, including patients and families, staff, and community health care providers. Candidate must be culturally competent and be able to thrive in a moderately paced, urgent need, complex, health care environment.
