Job summary

Midwest Health, Inc. is seeking a Clinical Support RN to serve as a primary clinical responder and escalation resource, providing real-time guidance, clinical decision-making, and care coordination to assist communities and provide support as needed.

This role collaborates with Executive Directors and RCCs to guide on level of care, document interactions, and participate in QAPI, ensuring high-quality resident care.

Qualifications

• LPN license, compact/multistate, required.
• Experience in clinical support or long-term care preferred.
• Ability to manage complex clinical situations.
• Strong communication with physicians, families, and staff.

Responsibilities

• Provide real-time clinical guidance as part of QA and respond to calls/alerts.
• Manage and document complex clinical situations (falls, changes in condition, behaviours).
• Advise on appropriate level of care and recommend resident interventions.
• Communicate with physicians, families, and staff.
• Document interactions in EMR or other software.
• Review, educate, and provide guidance on sentinel events and participate in QAPI.

• Manage and document complex clinical situations (falls, change in condition, behaviours)
• Advise on appropriate level of care and recommend for resident interventions
• Communicate with physicians, families, and staff
• Document interactions in EMR or other software
• Review, assess, educate, and provide guidance on senital events and participate in the QAPI program

This person have a compact/multistate Licensed Practical Nurse (LPN) license.
