Preferred Care Home Health in Naples, FL is seeking a Clinical Supervisor (RN) to lead our home health team and ensure high-quality patient care. You will supervise clinical staff, oversee assessments, care plans, and OASIS documentation while maintaining regulatory compliance.

In this role you’ll mentor nurses, coordinate with physicians and referral sources, and drive QA initiatives to improve outcomes and efficiency. Excellent leadership and communication are essential.

Qualifications

• Active RN license in Florida
• Graduate of accredited nursing program
• At least 2 years of home health experience including OASIS
• Strong understanding of home health operations and regulatory requirements
• Previous clinical leadership or supervisory experience preferred

Responsibilities

• Oversee patient assessments and care plans for an assigned caseload.
• Evaluate patient needs and ensure proper service levels; implement plans.
• Provide clinical supervision to nursing and other staff, including hiring and onboarding.
• Serve as liaison between patients, physicians, caregivers, referral sources, and payers.
• Ensure clinical documentation meets Medicare, state, and agency requirements.
• Conduct routine chart audits to monitor compliance and quality of care.
• Monitor visit utilization and care plan adherence; track outcomes.
• Lead QA and performance improvement initiatives.
• Build relationships with physicians, referral partners, and community organizations.

What You'll Do – Key Responsibilities

As our Clinical Supervisor, you will play an important leadership role in ensuring quality patient care, regulatory compliance, and clinical team performance.

• Oversee patient assessments, reassessments, and individualized plan-of-care development for an assigned caseload.
• Evaluate patient needs, determine appropriate levels of service, and ensure care plans are properly implemented and maintained.
• Provide clinical supervision and leadership to nursing and other clinical staff, including interviewing, hiring, onboarding, and staff development.
• Serve as a primary liaison between patients, physicians, caregivers, referral sources, and insurance payers.
• Ensure clinical documentation meets Medicare, state, and agency regulatory requirements.
• Conduct routine clinical record audits to monitor compliance, documentation accuracy, and quality of care.
• Monitor visit utilization, physician communication, care plan adherence, and overall clinical performance.
• Provide guidance and ongoing education to clinical staff regarding OASIS documentation, regulatory changes, and best practices.
• Lead quality assurance and performance improvement initiatives.
• Build and maintain positive relationships with physicians, referral partners, and community organizations.
• Promote efficient use of clinical resources while supporting agency operational and budgetary goals.

What We’re Looking For – Qualifications

• Current, active Registered Nurse (RN) license in the State of Florida.
• Graduate of an accredited nursing program.
• Minimum of 2 years of Home Health experience, including OASIS documentation.
• Strong understanding of home health clinical operations, documentation requirements, and regulatory compliance.
• Previous clinical leadership or supervisory experience preferred.
• Excellent communication, organizational, problem-solving, and clinical decision-making skills.
• Ability to manage multiple priorities, work independently, and thrive in a fast-paced environment.
• Strong attention to detail and commitment to quality patient care.
• Ability to assist with patient ambulation and transfers.
• Must successfully pass a health examination confirming freedom from communicable disease.
