Job summary

The Roads Home Health Care in South Florida is seeking an experienced Home Health RN Case Manager to lead patient care coordination for an assigned caseload. You will plan, coordinate, and document care from admission through discharge using clinical judgment to build individualized plans of care.

This role offers autonomy, strong patient relationships, and leadership of a multidisciplinary team, with opportunities to supervise staff and coordinate ongoing care and discharge planning.

Qualifications

• Active RN license in Florida
• Graduate of an accredited nursing program
• Minimum 1 year of acute care, home health, community health, or outpatient experience
• Current CPR certification
• Current health certificate and TB testing
• Valid Florida driver's license, reliable vehicle, and auto insurance

Responsibilities

• Complete admission assessments within 24 hours of referral or return home
• Develop and update individualized plans of care based on physician orders and patient needs
• Ensure timely OASIS assessments and documentation
• Assess and reassess patient pain and educate patients and families on pain management
• Administer medications safely to reduce errors
• Coordinate physician orders and communicate changes in patient condition
• Lead case conferences to review progress and adjust care plans
• Supervise home health aides and LPN/LVN staff and co-sign documentation
• Plan for safe discharge and coordinate referrals for continued or alternate care

About the Role

The Roads Home Health Care is seeking an experienced Home Health RN Case Manager to lead patient care coordination for an assigned caseload throughout South Florida. As a case manager, you will plan, coordinate, and document care for patients from admission through discharge, using your clinical judgment and home health expertise to build individualized plans of care. This is an excellent opportunity for a registered nurse who enjoys autonomy, patient relationships, and leading a multidisciplinary care team.

• Complete admission assessments within 24 hours of referral or a patient's return home
• Develop and update individualized plans of care based on physician orders and each patient's needs
• Ensure timely, accurate OASIS assessments and documentation
• Assess and reassess patients' pain levels, apply appropriate pain management techniques, and educate patients and families
• Administer medications safely, following best practices to reduce medication errors
• Obtain and coordinate physician orders, communicating promptly on changes in patient condition
• Lead case conferences with the care team to review patient progress and adjust plans of care
• Supervise home health aide and LPN/LVN staff, co-signing documentation as needed
• Plan for safe discharge and coordinate referrals for continued or alternative levels of care

Job Requirements

• Active, unencumbered Registered Nurse (RN) license in the State of Florida
• Graduate of an accredited school of nursing
• Minimum one year of recent experience in acute care, home health, community health, or an outpatient setting
• Current CPR certification
• Current health certificate/physical examination and TB testing
• Valid Florida driver's license, reliable vehicle, and current automobile insurance

Qualifications

• Strong clinical judgment and the ability to independently assess, plan, and adjust patient care
• Excellent verbal and written communication skills for coordinating with physicians, patients, and families
• Demonstrated leadership ability and comfort supervising other members of the care team
• Sensitivity to patients' cultural, ethnic, and personal preferences when delivering care and education
• Solid working knowledge of federal and state home health regulations
• Specialty certifications such as phlebotomy, IV therapy, or wound/ostomy care (ET) are a plus
