Job summary

Bristol Hospice is seeking a Clinical Supervisor to ensure hospice services are delivered appropriately and to supervise home care personnel in Everett, WA. This full-time position offers a day schedule and a competitive salary.

Qualifications

• A Registered Nurse license in good standing to practice professional nursing in the state.
• Must have hospice home care experience with two years of management or supervisory experience.
• Current CPR certification.
• Excellent observation, verbal, and written communication skills.
• A valid driver’s license.

Responsibilities

• Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs
• Assign appropriate hospice personnel to referral case as needed
• Confer with Medical Director and Attending Physician regarding any questions as to an individual’s eligibility for services
• Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians
• Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course
• Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services
• Available, during operating hours, to assist clinicians as appropriate
• Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program
• Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals
• Assist in the orientation of new employees, as needed
• Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in‑service and continuing education programs
• Assist in the formulation, revising, implementing and evaluating organization policies, procedures, goals and objectives – both short- and long‑range
• Comply with accepted professional standards and principles
• Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs
• Participate in public relation and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system
• Promote customer service orientation to all hospice personnel
• Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program
• Perform other duties and activities as delegated by the Director of Patient Care Service
• Additional duties as assigned
