Overview

Our Clinical Supervisors are responsible for ensuring the care and services are delivered appropriately and for supervising clinical personnel.

Responsibilities
  • Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs.
  • Assign appropriate hospice personnel to referral case as needed.
  • Confer with Medical Director and Attending Physician regarding any questions as to an individual’s eligibility for services.
  • Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians.
  • Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
  • Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services.
  • Be available, during operating hours, to assist clinicians as appropriate.
  • Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program.
  • Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals.
  • Assist in the orientation of new employees, as needed.
  • Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in‑service and continuing education programs.
  • Assist in formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives – both short‑ and long‑range.
  • Comply with accepted professional standards and principles.
  • Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs.
  • Participate in public relations and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system.
  • Promote customer service orientation to all hospice personnel.
  • Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program.
  • Perform other duties and activities as delegated by the Director of Patient Care Service.
  • Perform additional duties as assigned.
Qualifications
  • A Registered Nurse license in good standing to practice professional nursing in the state.
  • Must have previous experience in a hospice home care setting with at least two years of management or supervisory experience.
  • Enthusiasm for providing the best possible care to patients and their families.
  • Understanding of the hospice philosophy and issues of death and dying.
  • Excellent observation, verbal, and written communication skills.
  • Valid driver’s license.
  • Current CPR certification.
