Job summary

Bristol Hospice – High Desert in Klamath Falls, OR is seeking a Clinical Supervisor to ensure clinical services are delivered effectively and to supervise clinical staff. This on-site role focuses on patient care quality and operational excellence within the Klamath County area.

The successful candidate will hold an active RN license, have hospice home care supervisory experience, and demonstrate strong communication and leadership skills.

Qualifications

  • Licensed Registered Nurse in good standing to practice in the state.
  • Two years of management or supervisory experience in hospice home care.
  • Strong compassion for patients and families.
  • Understanding of hospice philosophy and death/dying issues.
  • Excellent observation, verbal, and written communication skills.
  • Valid driver’s license.
  • Current CPR certification.

Responsibilities

  • Receive case referrals and review patient information to determine hospice needs.
  • Assign hospice personnel to referral cases as needed.
  • Consult with Medical Director and Attending Physician on eligibility for services.
  • Review and evaluate cases through home visits, conferences, records review, and clinician services.
  • Review patient diagnoses, prognosis, medications, and clinical course.
  • Guide clinicians to improve performance and quality of home care delivery.
  • Be available during operating hours to assist clinicians as appropriate.
  • Conduct clinical record reviews and communicate findings to Clinical Director and hospice personnel.
  • Assist in screening and interviewing new employees and make recommendations for employment.
  • Assist in orientation of new employees as needed.
  • Assist Director of Patient Care Services in planning, implementing and evaluating education programs.
  • Help formulate and evaluate organization policies, procedures, goals and objectives.
  • Ensure compliance with professional standards and principles.
  • Assess community health needs and help develop plans to meet needs.
  • Participate in public relations and community activities promoting the organization’s role.
  • Promote customer service orientation to hospice personnel.
  • Participate in quality improvement teams as directed by leadership.

This position is based out of our Klamath Falls, OR office and supports hospice operations throughout the Klamath County area.

On An Average Day You Will

(Includes, but not limited to)

  • Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs
  • Assign appropriate hospice personnel to referral case as needed
  • Confer with Medical Director and Attending Physician regarding any questions as to an individual’s eligibility for services
  • Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians
  • Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course
  • Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services
  • Available, during operating hours, to assist clinicians as appropriate
  • Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program
  • Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals
  • Assist in the orientation of new employees, as needed
  • Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs
  • Assist in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives – both short- and long-range
  • Comply with accepted professional standards and principles
  • Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs
  • Participate in public relation and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system
  • Promote customer service orientation to all hospice personnel
  • Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program
  • Perform other duties and activities as delegated by the Director of Patient Care Service
  • Additional duties as assigned
Qualifications
  • A Registered Nurse license in good standing to practice professional nursing in the state
  • Must have previous experience in a hospice home care setting with two years of management or supervisory experience
  • An enthusiasm for providing the best possible care to patients and their families
  • Understanding of the hospice philosophy and issues of death and dying
  • Excellent observation, verbal, and written communication skills
  • A valid driver’s license
  • Current CPR certification
