Job summary
RADIAS Health is seeking a Mental Practitioner Supervisor to oversee a four-resident group home within the Residential Support Services program. You will provide supervision, clinical direction, and coordinate care with the treatment team, family members, and community professionals.

Qualifications
- Associate's degree in human services + 2 years of full-time work experience
- Bachelor's degree in human services + 1 year of full-time work experience
- Must have car, auto insurance and valid driver's license
Responsibilities
- Coordinate client care, including quarterly case conferences with the case manager and treatment team
- Develop, implement and review quarterly service plans and related behavior plans
- Coordinate and schedule client medical appointments and arrange transportation
- Identify individuals prone to behavior emergencies and develop additional programming
- Assist with client admission and discharge processes and connect clients to community resources
- Lead weekly house staff meetings and document meeting minutes
- Administer medications under nurse supervision and ensure medication sheets are updated
- Attend agency meetings and participate in clinically oriented groups
