A leading hospice provider in Ohio seeks a Clinical Supervisor to ensure quality patient care and oversee clinical personnel. The role involves evaluating cases, guiding clinicians, and participating in community health initiatives. The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor oversees the delivery of care and services, supervising clinical personnel to ensure quality and compliance.
Receive case referrals, review patient information, including home visits, to determine hospice/home care needs, and assign appropriate personnel. Confer with the MEDICAL DIRECTOR regarding eligibility questions.
Evaluate each case through home visits, conferences, record reviews, and service assessments. Guide clinicians to improve performance and ensure quality care. Be available during operating hours to assist clinicians.
Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.
Participate in case conferences with hospice personnel to coordinate care and discuss interdisciplinary involvement.
Conduct quarterly record reviews, communicate findings and recommendations to the Clinical Director and staff.
Assist in screening and interviewing new employees, make hiring recommendations, and help in orientation.
Support the Clinical Director and supervisory team in planning, implementing, and evaluating education programs and organizational policies.
Ensure compliance with professional standards and principles.
Assess community health needs and develop plans or programs to meet these needs. Engage in community activities to promote the organization’s role and foster customer service orientation.
Participate actively in quality assessment and performance improvement initiatives.
Registered nurse with current licensure in the state.
BSN degree from an NLN-accredited school preferred.
Experience in hospice/home care and at least two years in management or supervision.
Must have a valid driver’s license, insured vehicle, and current CPR certification.
Ability to work within an interdisciplinary team, with excellent observation, judgment, and communication skills. Understanding of hospice philosophy, death/dying issues, and pain management principles.
