Job summary

Qualifications
- Graduate of an approved school of professional nursing.
- Licensed in the state where currently practice, or as per nurse licensure rules.
- At least two years of nursing experience.
- One year of home health or hospice experience.
- Knowledge of federal, state, and local laws governing home care operations.
- Basic technology skills including operation of a mobile device.
Responsibilities
- Coordinate the overall delivery of home health services to patients according to standards and procedures.
- Review and approve patient information submitted at start of care, recertification, resumption of care, or evaluation visit.
- Facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
