Job summary

First Day Homecare is hiring a Registered Nurse supervisor to lead client care teams across Utah. You will conduct in-home visits, supervise staff, and coordinate care with physicians and families to ensure the highest quality outcomes.

The role requires an active RN license in the state and 2+ years of home health experience. BSN is preferred. Flexible work-from-home options may be available, with on-site visits in Draper, UT and surrounding areas.

Qualifications

• Graduate of an accredited nursing program; RN license in state(s) of practice.
• BSN preferred and beneficial for supervisory role.
• Strong communication and interpersonal skills required.
• Knowledge of federal and state home health regulations and documentation requirements.

Responsibilities

• Conduct in-home client visits for start-of-care assessments and supervisory visits.
• Be available during business hours and on-call as needed.
• Receive referrals, assess service needs, and assign clinicians to cases.
• Meet with clinical staff to provide guidance and evaluate progress.
• Attend clinical meetings to coordinate care and participate in on-call rotation.
• Educate clients and families on healthcare resources and policies.
• Oversee clinical documentation accuracy and perform quarterly chart reviews.
• Assist with staff hiring, orientation, training, and annual performance evaluations.

• The position may require occasional weekend or evening visits to client homes and participation in a clinical on-call rotation (phone support only).
• Our office is in Draper Ut, but we provide services throughout the State of Utah

Job Summary

Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of services of the Agency and all applicable policies, standards, laws, and state Nurse Practice Act.

Provide Supervision to:

All direct care staff

Responsibilities

• Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
• Be available during business hours and on-call as needed to support quality care delivery.
• Receive referrals, assess service needs, and assign appropriate clinicians to cases.
• Meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
• Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
• Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
• Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
• Assist with staff hiring, orientation, training, and conducting annual performance evaluations.

Qualifications

• Graduate of an accredited school of professional nursing. BSN preferred.
• Current license as a Registered Nurse in the state(s) of practice.
• A minimum of two (2) years of homecare experience is required. Supervisory experience is preferred.
• Excellent verbal and written communication skills and strong interpersonal skills.
• Knowledge of federal and state regulations regarding home health.
• Knowledge of reimbursement sources and documentation requirements within home health care.
• Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
• Current CPR certification.

Compensation: $43.00 - $45.00 per hour
