Job summary

The Palms Home Care is seeking an experienced Clinical Supervisor to lead our clinical team and ensure delivery of high-quality patient care. You will supervise staff, coordinate care plans, and monitor outcomes to uphold clinical excellence.

RN licensure is required, with at least one year of supervisory experience in home health. Strong leadership and collaboration across interdisciplinary teams are essential for success in this role.

Qualifications

• Familiar with OASIS and care documentation.
• Experience with EMR systems such as Kinnser.

Job description

We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
• Conduct regular performance evaluations and provide constructive feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
• Performs other duties and activities as delegated by the Clinical Director.

Qualifications
