Job summary

Diamond Care Group seeks an experienced Clinical Supervisor for its Palms Home Care team in Orlando. You will lead clinical staff, oversee patient care plans, and ensure adherence to best practices.

The role requires RN licensure and prior home health supervisory experience. Responsibilities include supervising clinicians, evaluating performance, and coordinating with interdisciplinary teams to optimize patient outcomes.

Qualifications

• RN license in good standing.
• At least one year of management or supervisory experience in the home health setting.
• Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination.
• Experience using EMR systems such as Kinnser.
• Knowledge of clinical best practices and regulatory standards.
• Commitment to maintaining patient confidentiality and ethical standards.

We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.

• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.

• Conduct regular performance evaluations and provide constructive feedback to staff.

• Facilitate clinical training and professional development opportunities.

• Collaborate with interdisciplinary teams to develop and implement patient care plans.

• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.

• Assist in resolving clinical issues and addressing staff concerns.

• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.

• Performs other duties and activities as delegated by the Clinical Director.
