Job summary

Diamond Care Group in Port Charlotte, FL is seeking a dedicated Clinical Supervisor for our Palms Home Care team. You will lead clinical staff, coordinate patient care plans, and ensure high standards of home health delivery.

Qualifications include RN licensure and at least one year of supervisory experience in home health; familiarity with OASIS, EMR such as Kinnser; strong leadership and confidentiality.

Qualifications

• RN licensure in good standing across applicable states.
• Minimum of 1 year in a supervisory role within home health.
• Familiarity with OASIS and care documentation requirements.
• Experience using EMR systems such as Kinnser.
• Knowledge of clinical best practices and regulatory standards.

Responsibilities

• Review patient information and assign appropriate clinicians to cases.
• Review clinical diagnoses, medications, procedures and course.
• Supervise and mentor staff including therapists and counselors.
• Conduct regular performance evaluations and provide feedback.
• Facilitate clinical training and professional development.
• Collaborate with interdisciplinary teams to develop care plans.
• Monitor outcomes and ensure adherence to guidelines.
• Assist in resolving clinical issues and staff concerns.
• Participate in screening and interviewing new personnel.
• Perform duties delegated by the Clinical Director.

Job description

We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
• Conduct regular performance evaluations and provide constructive feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
• Performs other duties and activities as delegated by the Clinical Director.

Qualifications

• RN licensure in good standing.
• At least one year of management or supervisory experience in the home health setting. Strong leadership and interpersonal skills.
• Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination.
• Experience using electronic medical record (EMR) systems such as Kinnser.
• Knowledge of clinical best practices and regulatory standards.
