Description
Constellation Health Services is seeking a Clinical Supervisor for our Certified Home Health Program. The Clinical Supervisor is responsible for and accountable for delivering care consistent with the philosophy, goals, policies, and procedures of Constellation Health Services.

Responsibilities:
- Plans, develops, facilitates, and evaluates the implementation of multi-disciplinary services including Skilled Nursing, Physical Therapy, Occupational Therapy, SLP, Medical Social Work, and Home Health Aide in patients’ homes.
- Provides leadership and supervision to multi-disciplinary teams.
- Assigns, supervises, guides, and instructs staff to ensure high-quality services, accurate documentation, and participation in patient case conferences based on educational background and clinical competence.
- Ensures coordination of Constellation Home Care nursing services with other internal and external services.
- Determines staffing needs to meet objectives.
- Assists with recruiting, hiring, supervising, and terminating staff.
- Coordinates coverage for evenings, nights, and weekends as needed.
- Accepts on-call responsibilities as scheduled.
- Demonstrates mastery of State regulations and Medicare standards.
- Performs other duties as assigned.

Qualifications
- Registered Nurse (RN) with the required licensure, education, training, and qualifications as per state regulations.
- Minimum of three (3) years of full-time clinical experience, including at least one year in a home health or community health setting involving care of the sick at home.
- Must be a licensed driver with insured, well-maintained vehicle.
- Proven ability to work within an interdisciplinary team.
- Adheres to professional standards and practices.
- Excellent observation, judgment, and communication skills.
- Understanding of pain and symptom management principles.
